ZACHARY M. ZWOLAK, DO, FAAFP

Dr. Zwolak earned his BA from Franklin & Marshall College in Biology & French. He then studied osteopathic medicine at the Lake Erie College of Osteopathic Medicine followed by internal medicine and general surgery internships. He completed his Family Medicine Residency at St. Luke’s Hospital in Bethlehem, PA and advanced Obstetrics Fellowship at Natividad Medical Center in Salinas, CA.

He cares for addiction across the breadth of life from pregnancy to adults in the outpatient and hospital settings. Dr. Zwolak is board certified in family medicine and a Fellow of the American Academy of Family Physicians in recognition for contributions to the speciality. He is board eligible for Addiction Medicine and will be completing testing in the Fall. Dr. Zwolak is assistant director of the Ventura County Primary Care Addiction Medicine Fellowship training the future physicians in Addiction Medicine.

Dr. Zwolak has published chapters in several medical references, academic poster presentations and medical journal articles. He was awarded the Family Medicine Teaching Award for the Ventura County Family Medicine Residency in 2016. He was recently recognized in 2019 for a Medical Education Research Foundation Scholar for the California Society of Addiction Medicine for continued development of education in the field.

His interests include camping, travel, obstacle races and spending time with is two children and wife.
